君,周 琳   [ 摘要] 目的 分析北京市石景山区2010—2017 年中小学校和托幼园所集中发热疫情的流行特征,为疫情防控提供科 学依据。方法 采用描述性流行病学研究方法,对中小学校和托幼园所集中发热疫情流行特点及病例临床表现、病原学检 测等进行分析。结果 2010—2017 年石景山区中小学校和托幼园所共报告集中发热疫情42 起,涉及27 所学校和托幼园所, 疫情发生率为18.37%;报告病例1059 例,平均发病率为32.66%。疫情主要集中在11 月至次年1 月,报告疫情数占总疫 情数的85.71%。托幼园所、小学和中学疫情分别占35.71%、42.86% 和21.43%。托幼儿童、小学生和中学生发病率分别为 46.81%、24.87% 和12.49%。发热病例中体温达到38.5 ℃及以上的有584 例(55.15%),523 例(49.39%)的发热病例符合 流感样病例定义。患者临床表现以咳嗽、咽痛、头痛为主,托幼儿童和中小学生不同症状发生率不同。流感病毒引起的集中 发热疫情有34 起(80.95%),腺病毒和柯萨奇病毒引起的集中发热疫情有3 起(7.14%)。结论 石景山区中小学校和托 幼园所集中发热疫情冬季高发,主要由流感病毒引起,不同年份优势毒株型别不同,小学和托幼园所是疫情防控的重点。 
Abstract:
Objective To analyze the epidemiological characteristics of clusters of fever epidemic in kindergartens, primary schools and middle schools at Shijingshan District of Beijing from 2010 to 2017, and provide scientific evidence for the prevention and control of epidemic. Methods Epidemiological characteristics of clusters of fever epidemic, clinical manifestation and etiology detection of affected cases in primary and middle schools, as well as kindergartens were analyzed with descriptive epidemiological study. Results A total of 42 clusters of fever epidemic were reported in 27 primary and middle schools, kindergartens at Shijingshan District of Beijing from 2010 to 2017 and the morbidity was 18.37%; a total of 1059 cases were reported and the average incidence was 32.66%. The clusters of fever epidemic were mainly found during November to January next year, accounting for 85.71% of total clusters. The proportion of the clusters in kindergartens, primary and middle schools were 35.71%, 42.86% and 21.43%, respectively. The incidences for kindergarten children, primary and middle school students were 46.81%, 24.87% and 12.49%, respectively. Among the fever cases, 584 cases (55.15%) had a body temperature up to 38.5 ℃ or higher, and 523 cases (49.39%) conformed to the definition of influenzalike illness. The main clinical manifestations included cough, sore throat and headache, the incidences of different symptoms varied kindergarten children and primary/middle school students. There were 34 clusters caused by influenza virus (80.95%) and 3 clusters (7.14%) caused by adenovirus and Coxsackie virus. Conclusions The clusters of fever epidemic in kindergartens, primary schools and middle schools at Shijingshan District are mainly caused by influenza viruses and prevalent in winter. Dominant strain types are different in different years. The prevention and control of clusters should focus on kindergartens and primary schools.
