摘要:
目的 分析AIDS 相关性淋巴瘤(acquired immunodeficiency syndrome-related lymphoma, ARL)的病理结果、 分布部位及影像学特点,以提高对ARL 的认识。方法 回顾性分析本院2016 年1 月—2017 年11 月期间经病理结果确诊为 ARL 的27 例患者的临床资料,分析总结ARL 的特点。结果 27 例ARL 病例中以非霍奇金淋巴瘤(non-Hodgkin lymphoma, NHL)为主,其中主要包括弥漫性大B 细胞淋巴瘤及伯基特淋巴瘤;患者以中青年为主,结内多为多部位淋巴结肿大,结 外多发生于颅内、颌面部、咽部、腋窝、腹股沟区及胃肠道;ARL 的影像学结果显示结内病变为多部位淋巴结肿大,部分 可为典型“血管漂浮征”,结外病变主要表现为肿块性病变,其中中枢神经系统ARL 呈现出不均匀性花环状强化。结论  ARL 以NHL 为主,广泛累及全身淋巴系统及结外脏器,病变进程快,其临床病理及影像学诊断特点显著,可以为ARL 的 及时诊断和积极治疗提供有利依据。  
Abstract:
Objective To analyze the pathological results, distribution areas and imaging findings of acquired immunodeficiency syndrome-related lymphoma (ARL), and further promote the understanding of ARL. Methods Clinical data of 27 ARL patients diagnosed by pathology from January 2016 to November 2017 were retrospectively analyzed, ARL features were summarized. Results  The dominant pathological type of 27 ARL patients was non-Hodgkin lymphoma (NHL), including diffuse large B cell lymphoma and Burkitt lymphoma; ARL was prevalent among young and middle-aged patients. Nodal lymphoma was characterized by multiple sites of lymph node enlargement, extra-nodal lymphoma mainly occured in the intracranial part, oral and maxillofacial part, axilla, inguina and gastrointestinal tract; Imaging results of ARL display nodal lymphoma enhancement in multiple sites, of which was expressed as vessel floating sign; Extra-nodal lymphoma of ARL was mainly mass lesions and characterized by uneven garland-like enhancement in central nervous system. Conclusions The majority of ARL is NHL, extensively involving the system lymphatic system and extra-nodal organs, and shows a rapid progression. The significant characteristics of clinicopathological and imaging results contribute to beneficial evidence for the timely diagnosis and proactive treatment of ARL.
